March Madness and Covid-19

Are you talking about Remdesivir?

No, I'm talking about steroids (specifically decadron). Not a huge fan of remdesevir, though I'd probably still give it to a family member under the right circumstances if forced to make that decision today. Steroids and anticoagulation are much more essential (again, under the right circumstances).

By the way, did you see the price tag they’re putting on remdesivir?

$3,100 per patient, yikes!

https://www.npr.org/sections/health-shots/2020/06/29/884648842/remdesivir-priced-at-more-than-3-100-for-a-course-of-treatment

Money in the medical world is never as it appears, and the whole process needs to be simplified. That's a complicated topic I'll avoid for the moment.

What I will say, though, is that if Remdesevir reduces hospital and ICU length of stay as claimed by the trial, it will be worth the money (even though it wasn't shown to have a mortality benefit). I don't know the specifics off hand, but I think most would be astonished to see the bill that simply sitting in the hospital for a day racks up, so $3100 is pretty easily earned back. Additionally, if it's getting people out of the hospital quicker, it also helps with overcrowding.

Decadron is dirt cheap in comparison, though, and has a significant mortality benefit, so it's certainly my preference. You can use both together, though, so one doesn't necessarily preclude the other. If this pandemic hits in full force again, I doubt we'll have enough decadron to go around, but I think it's reasonable to use other forms of steroids at comparable dosing when those stores run low (prednisone, methylprednisolone, etc).

Thanks for the info!

16 states are either pausing or rolling back re-opening.... Kevin Stitt, Ron Desantis, and some of these other morons are really looking bad now. More and more Republicans who did nothing but downplay from their positions of leadership are now encouraging masks and precautions.

On average, more than 1,000 Americans die every day from the disease.

"You got a lot of people in your profession (the media) who waxed poetically for weeks and weeks about how Florida was gonna be just like New York. Just wait, Florida is gonna be next, you said. Well hell, we are now 8 weeks away from that and it hasn't happened!"
-Florida Governor Ron DeSantis (R) on May 20th, 2020

Fast forward a few weeks.....

Florida has recorded more than 5,000 new COVID-19 cases per day since last Wednesday. On Saturday, the state documented its highest single-day increase with 9,585 new cases... Today (Wednesday July 1st), they have reported nearly 7,000 new cases.

And Trump is basically absent here... He doesn't want to talk about Covid anymore. He's surrendered. His latest tweets as 1,000 Americans per day die, and thousands upon thousands get sick:

"NYC is cutting police funding!!!"

"Biden read his responses from a teleprompter!"

"The Russian bounty story is a complete hoax!"

"WOW! Record growth in Q2! Under Biden, your 401k's will plunge!!"

"My executive order to protect statues is in full force!"

"I will veto the bill of Elizabeth "Pocahantas" Warren that wants to change the names of military bases from Confederate heroes because we won two world wars with people trained in those bases!"

(you can't be ****ing serious with that logic, by the way... but he is)

This is just pure insanity.... and all of you people who support this guy will say "geez, why are you paying attention to what he says?".... "you shouldn't be listening to him"..... i don't care about new cases. I care about deaths and hospital capacities.
These "idiot" governors are taking advice from health professionals and weighing that to the effects of shutting stuff down again.

deaths are still going down in florida

also, by your own admission, nothing was ever really shut down. So it seems a bit hypocritical to ride governors for not closing things down again if they were never shut down in the first place

I'll add that i do think it is evident that masks need to be mandatory and we should take this serious.

But I honestly believe that things are not any worse now than they were when we first shut down. I think we missed a lot of people that were never tested back in March and April and that the true number of people that have had the virus are pretty consistant. Basically, the numbers now are pretty realistic and the numbers in March/April were undercounted

i don't care about new cases.

Then I guess there is nothing to discuss... If you don't care about the virus spreading, not sure there is much else to say.

They downplayed it.

And Florida (until this week) has not been reporting on hospitalizations related to Covid. We will see what's going on when those numbers come out. In Miami-Dade, over 1200 people are on the hospital for Covid-19, which is more than at any point.

Texas set a new record for Covid-19 hospitalizations every day for 2 straight weeks until Sunday and then set a new record on Monday.

I mean, I care that it is spreading. But at some point, you have to live with it if the deaths are not increasing. It is going to be a part of life.
Again, I'd like to go back and revisit the original goal of the lock downs....flattening the curve long enough for hospitals to get ready. That has been accomplished. People want to rewrite history and that is what I have a problem with. People are quick to criticize when they will never be in the position making the choices with more data available than you and I have behind a computer screen. Do you guys really think republicans want people to die and hte virus to spread?

People need to be smarter. Masks need to be in place. Large gatherings should still be limited.

But it is time to ride the wave

Texas set a new record for Covid-19 hospitalizations every day for 2 straight weeks until Sunday and then set a new record on Monday.

And last I checked, they still had plenty of room for surges.

To put it in perspective...the new hospitlization in texas yesterday was 419 people. that comes out to .01 people per thousand. This is comparative to 58 people in Oklahoma. It is alarming that this is being seen every day.

Total in the hospital is 5900. Yes this sounds high, but I have no idea how many is normal and what hte capacity is. The 5900 is equivelent to about 815 in oklahoma...so quite a bit more.

We should definitely be taking more precautions so we can limit it as much as possible.

well I sure am glad I can just disregard all these stories of worry from Houston doctors since we have you experts in epidemiology here.

Nothing really in there that disputes anything we've said

CAses are increasing.
Hospitals beds are increasing but not at capacity
Something needs to happen to slow it down.
Deaths, thankfully, are not increasing*

*Yes, we know it is a matter of time before deaths do start to increase

I heard there was a couple of taco stands which temporarily closed down in California because customers who did not want to wear masks were verbally assaulting the employees who were simply following the rules about wearing them when you walk up to them to order their food. Just when I thought we couldn’t possibly get more mentally weaker as a country, a bunch of people somehow lower the bar even more.

Here is a real simple concept that anyone with a brain can follow. If the place you are visiting requires face masks, either wear them or find someplace that doesn’t. There is no need to harass the employees who are simply following the rules laid out by their superiors. Will someone explain to me why this is such a hard concept for some to grasp?
