March Madness and Covid-19

I see they are touting a new steroid, Dexamethasone, as a great treatment for Covid-19.

Hearing anything on this, Steve?

It’s not a new steroid, as it’s used widely for other uses, but it’s “new” to the treatment of COVID. I haven’t heard of anyone in our practice area using it. Looks like This was a british study. We’ll see if it’s effective and begins to pop up in the states.

Correct, good clarification. The British study looked promising.

Convalescent plasma is another promising treatment, although not a magic bullet. Some of the studies in China and other areas were stopped because the pandemic waned. It’s been used in the states with COVID for about 2 months now, but it’s still too early to say definitively, although it shows promise.

We have given 2 units of CCP here. You obviously can't make any determinations based on 2 units but one patient recovered and one died. We are doing it as part of a study through the Mayo Clinic. Obviously 2 units isn't significant enough to draw any conclusions.

Steroids are not a new part of COVID treatment. They have been used since basically day 1 of COVID treatment, though with considerable controversy. I'm pretty firmly on the pro-steroid side, and have seen many patients do well with it. Others refuse to use it, citing mostly theoretical concerns. Evidence has thus far been almost entirely on the pro-steroids side, though it's far from a settled topic.

Dexamethasone has a few potential benefits over other steroids, but I primarily use it because of its once a day dosing (thus reducing nursing exposure).

New high for Oklahoma reported today (+228)
4 reported new deaths
around 20-30 new hospitalizations
positive test % up from 3.6 to 3.8

It'll be interesting to see if deaths increase in line with new cases. I have a feeling they will not, but we'll see.

I would guess not as well. LIke you or someone else said earlier, this new wave will probably be mostly younger people that are not high risk

Except for the fact that hospitalizations are going up. By the way, Tennessee now has it's highest rate of Covid-19 hospitalizations yet.

What point are you trying to say I am wrong about? That I think younger people will be more effected or that younger people are not high risk?

Hospitalizations are not deaths.

You may be right. I just think it makes sense that more younger people are going to be effected this time around

I am saying that increased hospitalizations will eventually lead to increased deaths, and that even though younger people are not the ones more likely to die, they will pass it on to others that are more likely to have adverse outcomes, this virus is highly contagious.

"top Italian doctor" turns out to be one doctor at one hospital, and the government doesn't agree with him.

"The government urged caution, saying it was far too soon to claim victory.
'Pending scientific evidence to support the thesis that the virus has disappeared ... I would invite those who say they are sure of it not to confuse Italians,' Sandra Zampa, an undersecretary at the health ministry, said in a statement."

We should probably wait until there is some evidence of this besides one doctor's opinion.
